Be a Partner for Your Community: A key to engaging local communities is understanding the issues that affect them the most. The community must view us as a partner that will be there for the long term with their concerns in mind.
Host Events, Trainings and Seminars: We host health fairs that includes nurses and educators to administer blood pressure and glucose checks, distribute preventive health education materials, provide consultation and maintenance measures for Cardiovascular Disease, Diabetes, Obesity and more. We provide monthly virtual and in-person education seminars and health awareness discussions.
Our Community Health Assessments (CHA) is a continuous process that helps us identify the strengths and barriers that impact the health of our communities. The information gathered drives our community-based programs and shapes our collaborative partnerships to create regional goals to improve health and create a culture of wellness.
